Short version
On a GLP-1 your stomach empties slowly, so a normal meal sits for hours and pushes acid up. The fixes are mechanical more than dietary: smaller meals, less fat, finish eating three hours before lying down, and stay upright after meals. Then the usual triggers: coffee, alcohol, chocolate, mint, tomato and citrus, mostly in the first weeks of each dose. You can still eat a full plate; it just has to be the right plate, earlier in the day.
Why it happens
Reflux happens when stomach contents push up past the valve at the top of the stomach. GLP-1s keep food in the stomach for hours longer than before, so the stomach stays full and under pressure long after you have finished eating. Fat slows emptying even further. Large meals, late meals and lying down after eating turn that pressure into heartburn, burping and a sour taste.
It is worst on peak days after a shot or in the first weeks of a new dose, and it usually eases as the dose settles. If you already had reflux before starting, a GLP-1 can make it noticeably worse, and that is worth telling your clinician.
What usually helps
Half portions, more often. A full plate on a slow stomach is the main cause. Three small meals and a protein snack beat two big ones.
Low fat for now. Grilled, baked, steamed, poached. No fried, creamy or cheese heavy meals in the first weeks of each dose.
Three hours between dinner and bed. The single most effective change for night time reflux.
Stay upright for two hours after eating. A slow walk helps; a couch does not.
Raise the head of the bed by 15 to 20 cm with blocks or a wedge if nights are bad. Extra pillows bend you at the waist and make it worse.
Drink between meals, not with them. A large drink adds volume to an already full stomach.
Know the triggers. Coffee, alcohol, carbonated drinks, chocolate, peppermint, tomato sauce, citrus, spicy food and onion. Cut them for two weeks, then add back one at a time and see which ones matter for you.
Loose waistbands. Pressure on a full stomach pushes acid up.

Low fat Greek yogurt
Ginger or chamomile tea, still water
Skip for now

Lying down within two hours of eating
A sample day
| Morning | Oatmeal with banana. Chamomile or ginger tea. |
| Mid morning | Low fat Greek yogurt. |
| Lunch | Grilled chicken, rice, steamed green beans. |
| Afternoon | Melon, or a few crackers with turkey. Water between meals. |
| Early dinner, by six | Poached fish or sliced chicken, mashed sweet potato, zucchini. A full plate is fine; stop at first fullness. |
| Evening | A slow walk. Water or herbal tea. Bed three hours after dinner, head of the bed raised. |
The portions are normal; what changes is the fat, the timing and the posture.
When to call your clinician
Difficulty or pain when swallowing, or food that feels stuck
Vomiting blood or material that looks like coffee grounds, or black stools
Chest pain or pressure, especially with breathlessness, sweating or pain in the arm or jaw: call 911, do not assume it is reflux
Heartburn most days for more than two weeks despite the changes above
Losing weight faster than expected because eating hurts
Common questions
Can I take an antacid?
Over the counter antacids and acid reducers are commonly used alongside GLP-1s. Ask your clinician which one; some need to be spaced from other medication, and on the Wegovy pill the empty stomach rule decides the timing.
Why is it worse at night?
Lying flat removes gravity while your stomach is still full from slow emptying. An early dinner and a raised bed head fix most night reflux.
Does it go away?
For most people it eases within a few weeks at a stable dose and flares briefly after each increase. If it persists for more than two weeks despite the changes above, raise it with your clinician.
Is burping normal?
Yes, burping is common on these medications, sometimes with an unpleasant sulfur smell. Smaller meals, less fat and no carbonated drinks reduce it. Burping with vomiting or severe pain needs a call.
Should I still eat before my injection?
Yes, a normal meal on shot day is fine for most people. The lighter, earlier eating starts the next day when the level rises.
